[B] Scope of Service:

(1) Vendor needs to provide brain injury case management service for the documenting service provision, record keeping and required reporting for all state-funded activities.
- Focus on providing opportunities for individuals with brain injury to function and become as independent as possible in community-based settings.
- Support people with brain injuries to transition to or be supported in, community integrated settings.
- Activities that develop or expand support groups; form relationships with community partners, identify, develop and refer to community resources, develop or improve access to resources, including, but not limited to, written and online-information.
- Activities that develop, facilitate, sponsor or contribute to activities that increase the community’s general knowledge of brain injury through print or broadcast media, health fairs, fundraising events, awareness activities and other community related events.
(2) A pre-bid conference will be held on January 24, 2023.
(3) All question must be submitted no later than January 27, 2023.
(4) A contract period will be for one year.
